What to send with a referral
The more of this that arrives with the first fax, the faster the patient gets their equipment. If something is missing we will call your office rather than sit on it.
A standard written order
Every insurance-covered item needs an order containing:
- Patient name or Medicare Beneficiary Identifier
- Date of the order
- A description of the item — general description, HCPCS code or brand and model
- Quantity, where it applies
- Treating practitioner’s name or NPI
- Treating practitioner’s signature
Chart notes supporting medical need
Coverage is decided on the medical record, not the order alone. Send the notes that show why the item is needed — diagnosis, functional limitation, what has already been tried, and any relevant test results.
A face-to-face encounter, where required
Certain items require a documented face-to-face visit within the six months before the order, plus a written order received before delivery. We will tell you if the item you are ordering falls into that group.
Where we deliver
We are licensed as a DME supplier in Colorado, and enrolled with Medicare Part B and Health First Colorado. Insurance-billed equipment, in-person delivery, setup, education and service are for Colorado residents.
Everyday and over-the-counter supplies ship nationwide on a self-pay basis. For a patient in another state who needs insurance-billed equipment, call us before anything is promised — we will tell you honestly whether we can serve them, and refer you on if we cannot.
What your patient gets
- A benefits check before anything is ordered, and a clear statement of their cost
- Delivery, and in Colorado, setup and training in the home
- Warranty and repair handling on equipment we provide
- Resupply contact before supplies run out
